Human immunodeficiency syndrome (HIV) is increasingly recognized as an illness that affects the whole family, and not just the individual. For many HIV infected individuals; families are often the source of most of the physical and emotional care. I am proposing to present a 30 minute talk on “One Stop Shop Model of Care for individuals and Families Living with HIV”. This Model of care is designed based the “Medical Home” concept; the state of art of the 21st century health care delivery system. This family-centered model of care focuses on integrating high quality health promotion for HIV infected and affected individuals and their families from infancy through adulthood individuals and families living with HIV, and ensures that their medical and non-medical needs are met. The presentation will touch on strategies for implementation of this concept in clinical setting.
